You reaffirm your commitment to the Synapse Pledge.

You will not attempt to establish the identity of or contact any of the data subjects.

You understand that the Data will not include personally identifiable information and that under no circumstances will any codes that may link these Data to Protected Health Information be given to you.

You confirm that if you inadvertently receive identifiable information or otherwise identify a subject, you will promptly notify the ACT by emailing act@sagebase.org.

You agree to retain control over the Data and to not disclose, release, sell, rent, lease, loan, or otherwise grant access to the Data to anyone not specified in your data access request.

You agree to establish appropriate administrative, technical, and physical safeguards to prevent unauthorized use of or access to the Data.

You will report any data misuse or breach of data security to ACT by emailing act@sagebase.org.

Publications generated using data accessed through the ARK Portal must acknowledge both the ARK Portal and the data contributor(s) as described in the study specific Acknowledgement Statements.
